Service Coordinator – Enliven
Coordinate person-centred home support services for older people, combining clinical expertise with service coordination in a supportive, values-led team.
At Enliven, we support older people to live independently and with dignity in their own homes and communities. We are seeking a Service Coordinator to join our Enliven team and play a key role in delivering high-quality, person-centred home and community support services.
This is a rewarding opportunity for a registered nurse who enjoys balancing clinical practice with coordination, relationship management and service improvement.
Te āhua o te mahi | The Role
Reporting to the Lead Service Coordinator you will support in the delivery of a flexible, restorative, community-based service that is driven by what matters most to the people we support. You will work closely with service users, whānau, Support Workers, allied health professionals and external stakeholders to ensure services are well coordinated, responsive and aligned with Enliven’s service model and contractual requirements.
Please note: this is a 9 month fixed term role (March to Sept), covering a parental leave.
Key responsibilities include:
Coordinating services to ensure service users receive the right support at the right time
Undertaking clinical assessments and reviews for non-complex clients
Developing and maintaining individual support plans focused on independence and wellbeing
Supporting rostering, scheduling and effective Support Worker–service user matching
Building strong relationships with NASC, DHBs, GPs and community partners
Contributing to reporting, quality systems and service improvement initiatives
Supporting staff development, engagement and a positive team culture
Working in a way that honours Tangata Whenua and promotes culturally appropriate practice

You will bring strong clinical capability alongside excellent organisation and relationship-building skills. You enjoy working collaboratively and are committed to delivering high-quality services for older people.
You will be:
A registered nurse (essential)
At least three years’ post-registration clinical experience
Experience in aged care, community health or rehabilitation settings
Strong assessment, documentation and care planning skills
Experience coordinating services and working with multidisciplinary teams
The ability to manage competing priorities in a fast-paced environment
Well-developed communication and problem-solving skills
