Our Enliven Service is a leading provider of home and community support for older people and those with disabilities or recovering from injury. Our team of more than 600 believe that our elderly, those living with dementia and other conditions should be able to maximise their independence and engage, socialise and be active in their communities.

This role will work across our Enliven Northern region, being the wider Auckland area.
As a Service Coordinator, you will ensure delivery of our client-driven and flexible community based support services to the highest standards. This involves:
- Initial assessment and regular reassessment of clients in their own home
- Working collaboratively with clients, family and other agencies
- Development of individual support plans, in accordance with the clients’ specific goal-based needs
- Leading, developing and managing a team of Support Workers to deliver the support and assistance to meet clients’ goals and needs

We would love to hear from you if you have :
- Have a current New Zealand Registered Nurse Practicing Certificate
- A proven track record in a health background
- Experience leading a team
- Ability to build relationships and network
- Ability to work independently but also part of a team
- Capability to prioritise and manage your time effectively, with well developed organisational skills
- An understanding of the restorative approach to service delivery in the home care based environment
